How to read Ukrainian Visa


  1. Place of visa issuance (San Francisco, Washington, D.C., New York or Chicago).

  2. Traveler's visa is valid from this date. It reads Day-Month-Year. Thus, visa is valid from December 1, 2003 (not January 12, 2003).

  3. Visa expiration date. Visa is valid until this date. It reads Day-Month-Year. Thus, visa is valid until June 1, 2004 (not January 6, 2004).

  4. Visa control number.

  5. Number of entries.

    • Одноразова (1) - single entry visa (one entry only).
    • Дворазова (2) - double entry visa (two entries only).
    • Баторазова (М) - multiple entry visa (unrestricted within visa duration).

  6. Type of visa:

    • Д - Diplomatic;
    • C (C-1, C-2, C-3) -Service;
    • Б - Business;
    • Л - Rescue service (workers) employees;
    • О - Student visa;
    • Н - Scholar visa;
    • М - Visa for mass media representatives;
    • Р - Religious visa;
    • Г - Humanitarian visa;
    • К - Cultural and Sport visa;
    • Т - Tourism;
    • П (П-1, П-2) - Private visa;
    • ІМ (ІМ-1, ІМ-2) - Immigration visa;
    • ОП -Crew member visa (engaged in transportation);
    • ТР (ТР-1, ТР-2) - Transit visa.

  7. Traveler's last name in Ukrainian and English (as it appears on your passport).

  8. Traveler's first name in Ukrainian and English (as it appears on your passport).

  9. Traveler's passport number.

  10. Traveler's sex.

  11. Traveler's date of birth.

  12. Traveler's nationality.

  13. Host in Ukraine.

ALL APPLICANTS SHOULD CAREFULLY READ THE VISA REQUIREMENTS
AND SUBMIT ALL THE REQUIRED DOCUMENTS.


ALL APPLICATION PAPERS MUST BE CORRECTLY AND COMPLETELY FILLED OUT
(as well as a proper payment attached)
. FAILURE TO COMPLY OR PROVIDING
INCORRECT OR MISLEADING INFORMATION WILL RESULT
IN THE RETURN OF THE APPLICATION UNPROCESSED.

The Consulate General of Ukraine in San Francisco
is open to the public Monday through Friday from 9:15 a.m. to 12:00 p.m.,
except on weekends, Ukrainian and American legal holidays.
Visa documents are accepted until 11:45 a.m. only.
